Jira here, Monday there, Excel and PowerPoint everywhere else. Nobody agreed on a system, so the PMO ends up chasing six different formats just to see one project.

One slide says the deadline moved. Another says it didn't. The steerco pack and the tracker show two different completion percentages for the same workstream. Nobody can say which one's right, including the person who built both.

Status meetings collect information instead of using it, so the real conversation never gets the room's time.

Same slide, same format, same layout, just newer numbers copied in by hand. Hours spent formatting information that was already sitting in a system somewhere.

A workstream flags a blocker on Tuesday. The team it affects finds out at Thursday's steerco. The gap between something being known and the right person hearing about it is where projects actually slip.

Green today doesn't mean current today, it might mean nobody's touched it in two weeks. Without a record of when data was last verified, "up to date" is a claim, not a fact.
